The Universal Screening Triage (SDQ, SRSS-IE) report displays identifiable, real-time data from all Universal Screening forms completed for participants.
The forms included are dependent on which versions are assigned to your organization and have at least one completed screen.
The report only displays information about screens that have been finalized and does not include forms that have not been started or are still in progress.
Dropdown/Selectable Filters:
You can use the dropdown/selectable filter options to narrow the report results to only show specific forms, periods, programs, grades, locations, screening groups, and scoring options.
Scoring options reported depend on the selection in the form filter. Otherwise, the default view will show the form listed first, completed across all participating programs and locations.

Download CSV
Once you have filtered the report based on your needs, you can export the entire report as a CSV using the Download CSV button found on the upper right-hand side.
