Instructions for Requesting Participant Merge

 

mdlogix has created a process for merging duplicate participants in bhworks. Before we can run the script, please follow the instructions below to update the records and to provide the necessary information to mdlogix.


Once the below steps are complete, please email support@mdlogix.com to submit a ticket. 

 

Client Tasks

  1. Identify the participants that need to be merged ("source" and "target" records). 
  2. If necessary, please manually update the target record’s profile from the source record. The information in the source record will be retained after the merge process. The information captured in the participant profile will vary based on your organization’s configuration.
    1. Name
    2. DOB
    3. Race
    4. Ethnicity
    5. Date of Death
    6. Reason for Death
    7. Gender
    8. Primary Insurance Carrier
    9. Medicaid ID
    10. Home Phone
    11. Cell Phone
    12. Other Phone
    13. Email
    14. Address
    15. Preferred Language
    16. Needs Interpreter
    17. Assistive Device
    18. Assigned Staff
    19. Username
    20. Location
    21. SSN
    22. Preferred Contact Method
    23. Veteran Status
    24. Parent/Guardian Contact Information

  3. Please send the merge request(s) to the support team (support@mdlogix.com) with the following information:
    1. 1st merge: 
    2. 2nd merge: 

 

Note: The “source” record will be merged into the “target” record. Profile information on the target will be maintained. The “source” record will be deleted. The merge script execution cannot be reversed. It is critical that the source and target records are accurately identified, reviewed, and confirmed before sending to mdlogix.
