bhworks Reports & Dashboards (Release Version 25.3)


The bhworks Platform’s Reports tab includes data available through reports and dashboards. The main difference between reports and dashboards is the display of the data. Reports are displayed in a table format only. Dashboards use visualizations (bar graphs, pie charts, metrics, tables, etc.) to display the data. Both reports and dashboards allow users to filter for specific data. Below is a list of all of the reports and dashboards available to organizations and users within this release.


Case Notes: Billing Review - Displays a tabular report of services provided to participants for a billing specialist to review. Report includes participant name, date, provider, program, service, duration, site, and exported status. Filters allow users to pull data based on date, participant name/ID, provider, program, billing integration, billing approval status, review status, and/or exported status. Users can download/export the data as a CSV.


Case Notes: My Case Notes Report - Displays a tabular report of a provider’s entered case notes for their review. Report includes participant name, ID, date, scheduled status, start time, service, funding source, billing code, location, mode, site, encounter status, notes status, program, cancellation reason, duration, language, interpreter, updated date, and case note ID. Filters allow users to pull data based on date, location, encounter status, note status, and/or program. Users can download/export the data as a CSV.


Case Notes Overview - Displays de-identified aggregate real-time data and provides an overview of case notes (services and activities) that have been entered into bhworks for an organization. Information here includes an overall count of case notes, unique participants, unique providers, and duration. Graphs breakdown the data by demographics (gender, race, ethnicity, and age), month (encounter date), service type, staff, service name, service strategy, and reasons to treat. Filters allow users to drill-down by program, district/organization, school/location, status, and funding source. 



Case Notes: Review - Displays a tabular report of participants and the services they received within a given time period. Services are entered via the case notes module. Report includes participant name, date, provider, program, note status, service, encounter status, duration, canceled reason (if applicable), site, and billable status. Filters allow users to pull data based on date, provider, encounter status, note status, and/or billable status. Users can download/export the data as a CSV. 


Case Notes: Services Report - Displays a tabular report of participants and the services they received within a given time period. Services are entered via the case notes module. Report includes participant name, ID, date, start time, service, billing code, provider, location, site, encounter status, note status, program, duration, language, interpreter, date updated, and case note ID. Filters allow users to pull data based on date, provider, location, encounter status, note status, and/or program. Users can download/export the data as a CSV. 


Case Notes: Supervisor Log - Displays a tabular report of a provider’s entered case notes that are under supervision organized by participants. Report includes participant name, date, program, note status, approval status, reviewed by, reviewed date, service type, encounter status, service duration, canceled reason, group size, site of encounter participant progress, reason to treat, and billable status. Filters allow users to pull data based on date, program, and/or approval status. Default approval status is set to ‘rejected’. Clicking on the service line allows the supervisee to review the entire service note, view comments entered by their supervisor, and edit the note if rejected. The services shown in this report are only viewable by the person under supervision.  


Case Notes: Supervisor Review Report - Displays a tabular report of a provider’s entered case notes for supervisor review organized by participant. Report includes participant name, date, provider, program, note status, approval status, review date, service type, encounter status, service duration, canceled reason, group size, site of encounter participant progress, reason to treat, and billable status. Filters allow users to pull data based on date, provider, program, and/or approval status. Default approval status is set to ‘pending’. Clicking on the service line allows the supervisor to review the entire service note, add comments, and approve or reject. The services shown in this report are only viewable by the person assigned as the supervisor to the staff user.  


Discharge Summary: Overview - Displays de-identified aggregate data on the number of Discharge Summary records. The data includes counts for total discharge summaries as well as a count of unique participants. Graphical breakdowns display the data by demographics (gender, race, ethnicity, and age), month (finalized date) and reason for discharge. Filters allow users to drill-down by program, district/organization, and school/location.



Forms: Data Export - Provides a function to export the forms data entered into bhworks for an organization. Users can specify the forms data they would like to export by applying filters by organization, program, date range, and forms data with options to include participant name, demographics, MRN / IDs, and bhworks ID and to send an email notification when the report is ready for download. Users can download/export the data as a CSV.


Indirect Services: Overview - Displays de-identified aggregate data on the indirect services provided by an organization. Indirect services are those not considered direct participant services (SEL lessons, consultation, training, etc.). Information here includes an overall count of indirect service entries, attendees, and duration. Graphs breakdown the entries and attendee data by month (service date), grade, staff, and service name. Filters allow users to drill-down by program, district/organization, school/location, service type/category, and service status. 



Indirect Services: Report - Displays a tabular report of indirect services provided within a given time period. Report includes date, start time, end time, service, program, funding source, location, grade, provider, status, duration, number of attendees, and description of indirect service. Filters allow users to pull data based on date, organization, program, provider, location, and/or status. Users can download/export the data as a CSV.


Participants: Episodes of Care Displays a tabular report of participants enrolled in a program and their episodes of care. Report includes participant first name, last name, middle name, primary ID, bhworks ID, alias, program, episode number, status, start date, end date, duration, age at start, date of birth, gender, race, and ethnicity. Filters allow users to pull data based on the program. Users can download/export the data as a CSV.


Participants: Program Enrollment Report Displays a tabular report of participants enrolled in a program. Report includes participant first name, last name, middle name, primary ID, bhworks ID, alias, date of birth, gender, race, ethnicity, address, organization, location, email, created date, program assignment date, program, and assigned staff. Filters allow users to pull data based on the program. Users can download/export the data as a CSV.


Providers: Case Load Report Displays a tabular report of provider case loads (list of participant(s) assigned to the provider. Report includes provider name, participant name(s), location, episode of care status, and total assigned participants. Filters allow users to pull data based on provider organization, program, participant organization, provider, and/or episode of care status. Users can download/export the data as a CSV. 


Providers: Hours Report - Displays a tabular report of providers and total time spent providing services (direct & indirect) within a given time period. Report includes provider name, date, service, type, note status, location, grade, attendees, and duration. Filters allow users to pull data based on date, organization, program, provider, location, and/or note status. Users can download/export the data as a CSV. 


Referrals: Overview - Displays de-identified aggregate data on the number of referrals (incoming and outgoing) for an organization. The data includes counts for total referrals as well as a count of unique participants. Graphical breakdowns display the data by demographics (gender, race, ethnicity, and age), month (referral date), primary referral reason, referral made by, referral made to, started services status, reason not started services, entered by user, and acknowledged by user. Filters allow users to drill-down by referred to, referred by, referral status, and started services status



Referrals: Referral Details - Displays de-identified aggregate data on the number of Referral Details records (entered on the Referral Details form) when the referral was made. The data includes counts for total referral details forms as well as a count of unique participants. Graphical breakdowns display the data by demographics (gender, race, ethnicity, and age), month (finalized date), grade, person entering the referral, emancipated minor, secondary referral reasons, concerns regarding behavior, and current supports. Filters allow users to drill-down by program, district/organization, school/location, and task name.



Referrals: Referred Participants Report - Displays a tabular report of participants who were referred by an organization and/or program within a given time period. Report includes participant name, ID, date of birth, gender, race, ethnicity, date referred, referred by, entered by, referred to, referral status, started services, and reason not started. Filters allow users to pull data based on the referred by organization/program, referral date, and/or date. Users can download/export the data as a CSV.


Screening Activity: BHS (Adolescent/Adult) - Displays de-identified aggregate data on the number of Behavioral Health Screen (BHS) screening records. The data includes counts for total screenings as well as a count of unique participants. Graphical breakdowns display the data by demographics (gender, race, ethnicity, and age), month (finalized date), count of screenings that met the at-risk thresholds, and average domain scores by time period (for those using repeated measures). For each domain, there are also displays of average scores,  total scores, and a trend line by month. Filters allow users to drill-down by program, district/organization, school/location, and task name.



Screening Activity: BHS (Child) - Displays de-identified aggregate data on the number of Behavioral Health Screen Child version (BHS-C) screening records. The data includes counts for total screenings as well as a count of unique participants. Graphical breakdowns display the data by demographics (gender, race, ethnicity, and age), month (finalized date), count of screenings that met the at-risk thresholds, and average domain scores by time period (for those using repeated measures). For each domain, there are also displays of average scores,  total scores, and a trend line by month. Filters allow users to drill-down by program, district/organization, school/location, and task name.


Screening Activity: C-SSRS - Displays de-identified aggregate data on the number of Columbia-Suicide Severity Rating Scale (C-SSRS) screening records. The data includes counts for total screenings as well as a count of unique participants. Graphical breakdowns display the data by demographics (gender, race, ethnicity, and age), month (finalized date), risk level scores, and a breakdown of each question’s responses. Filters allow users to drill-down by program, district/organization, school/location, and task name.



Screening Activity: CRAFFT 2.1/N - Displays de-identified aggregate data on the number of CRAFFT 2.1 & CRAFFT 2.1N (tobacco/nicotine) screening records. The data includes counts for total screenings as well as a count of unique participants. Graphical breakdowns display the data by demographics (gender, race, ethnicity, and age), month (finalized date), average score, total scores, risk level scores, and a breakdown of each question’s responses. Filters allow users to drill-down by program, district/organization, school/location, and task name.

Graphical breakdowns display the data by demographics (gender, race, ethnicity, and age), month (finalized date), average scores in each category/area, and a breakdown of each question’s responses within each category/area. Filters allow users to drill-down by program, district/organization, and school/location.



Screening Activity: SECA (Short Form) - Displays de-identified aggregate data on the number of Social Emotional Competency Assessment (SECA) Short Form screening records. The data includes counts for total screenings as well as a count of unique participants. Graphical breakdowns display the data by demographics (gender, race, ethnicity, and age), month (finalized date), an overall average score, and a breakdown of each question’s responses within each category/area. Filters allow users to drill-down by program, district/organization, and school/location.



Screening Activity: SECA Universal Screening - Displays de-identified aggregate real-time universal screening data on the Social Emotional Competency Assessment (SECA) by category, percentage graphs and tables of each answer totals, and bar graph trends of each category through each universal screening over the school year. Customizable filters by organization, location, and program. 



Tasks: Overview (Completed) Displays de-identified aggregate data on the number of completed tasks (includes electronic forms as well as non-form tasks). The data includes counts for total tasks (completed) as well as a count of unique participants. Graphical breakdowns display the data by month (completed date), task name, and task list name. Filters allow users to drill-down by program, school/location, and task name.



Tasks: Report - Displays a tabular report of participants with tasks. Report includes participant name, ID, location, task name, status, created date, started date, due date, participant notified status, completed date, completed by, and manually status. Filters allow users to pull data based on task name, participant name, tasks status, staff, due date, program, and completed manually status. Users can download/export the data as a CSV.


Tasks: Report (Completed) - Displays a tabular report of completed tasks within a time period. Report includes task name, program/location, ID, participant name, date of birth, completed at date, completed by, and completed manually status. Filters allow users to pull data based on program, task name, completed manually status, and date. Users can download/export the data as a CSV.


Users: Report - Displays a tabular report of the staff user accounts within an organization. Report includes staff name, email, username, status, created at, last login, organization(s), role(s), program(s), funding source(s), primary funding source, credentials, and report(s). Filters allow users to pull data based on organization. Users can download/export the data as a CSV.
